Manual Therapy in Middletown, DE
Hands-On Treatment. Real Results.
Passive treatment alone doesn't solve athletic injuries. Manual therapy at Top Tier is used to reduce pain, restore range of motion, and help your body tolerate and benefit from the active rehab work that actually creates lasting change.

If Manual Therapy Is All You're Getting, You Deserve Better.
Passive Treatment Alone Won't Fix an Athletic Injury
Heat packs and manual work can ease the pain temporarily. But without identifying what's driving the injury and addressing it through movement and exercise, the same issues keep coming back.
Dry Needling Is a Tool, Not a Treatment Plan
Dry needling can be an effective clinical adjunct when used by a skilled provider. But it doesn't address the underlying movement dysfunction. We use it within sessions as one tool among many, not as the primary intervention.
Generic Hands-On Work Misses the Root Cause
Manual therapy applied without a thorough assessment can provide temporary relief without lasting change. The hands-on work here is always targeted, always purposeful, and always paired with an active rehab component.
How Manual Therapy Fits Into the Plan
No passive-treatment-only sessions. The manual work we do is purposeful, targeted to specific tissues and joints based on what your assessment reveals, and followed by active rehabilitation to reinforce the changes made.

When Manual Therapy Is Part of the Conversation

Frequently Asked Questions
Have questions about manual therapy? Here is what clients ask us most.
Is manual therapy the same as massage?
No. Manual therapy at the clinical level includes joint mobilization, manipulation, and soft tissue techniques applied as part of a specific plan of care. The goal isn't relaxation. It's restoring joint mechanics, reducing tissue restriction, and improving the way your body moves so it can tolerate and benefit from therapeutic exercise.
What is dry needling, and is it painful?
Dry needling uses a thin filiform needle to stimulate myofascial trigger points and address pain and movement impairments. It's different from acupuncture and is used as a clinical adjunct within sessions, not as a standalone treatment. Sensation varies by person and location, but most clients describe it as a brief, tolerable muscle response.
Do I need a referral to get manual therapy?
No referral is needed. We are a direct-access cash-based practice, so you can book a discovery call and come in without going through your physician first. We will always communicate with your care team if you have an existing treatment relationship we should know about.
Will I receive manual therapy at every session?
Not necessarily. Manual therapy is used when it's clinically indicated, not as a default. Some sessions focus entirely on therapeutic exercise and movement work. The goal is to progress you toward independent, lasting change, not to create ongoing dependence on hands-on treatment.
Can manual therapy help me avoid surgery?
In many situations, hands-on treatment combined with therapeutic exercise is an effective first step before more invasive options. We help you understand what's driving your symptoms and what can be addressed conservatively. We'll always be honest if your situation warrants a referral to a specialist.
