Performance Physical Therapy in Middletown, DE
Built for Athletes Who Refuse to Quit
Traditional PT keeps you waiting in line, hands you to an aide, and sends you home with a generic exercise sheet. We do the opposite: one-on-one care, direct access to your provider, and a plan built around what your body actually needs.

When You Keep Settling for Rest, the Problem Only Gets Worse
You've Been Told to Just Rest
Rest alone doesn't fix what's driving the pain. If the root cause isn't addressed, the same injury keeps coming back every time you load up again.
Generic PT Left You with More Questions Than Answers
Cookie-cutter programs and aide-led sessions don't give you the clarity you need. You deserve to understand what's wrong and what to do about it.
You're Afraid the Injury Means Stopping Forever
It doesn't. Most athletes and active adults don't need surgery or meds. They need a provider who understands how to keep them moving through recovery.
What Makes Performance PT at Top Tier Different
No aides. No techs. No shared appointment time. Just you, your physical therapist, and a plan of care that actually reflects what's going on with your body.

Injuries and Issues We Work With
These aren't separate service lines. They're the most common reasons athletes and active adults walk through our door. Regardless of where the pain is, the approach is the same: understand what's driving it, build a plan to address it, and keep you moving through the process.

Frequently Asked Questions
Still have questions? Here is what athletes ask us most before getting started.
Do you accept insurance?
Top Tier Physical Therapy is an out-of-network provider. We do not bill insurance directly. You pay at the time of service, and we can provide a superbill on request that you may submit to your insurance carrier for possible out-of-network reimbursement. We also accept HSA and FSA funds.
Why do you only see clients once a week?
Most clients are seen approximately once per week, but frequency depends on your needs, goals, and where you are starting. In some cases, we may recommend more than one visit per week early on to build momentum, manage symptoms, or work through a more acute issue. Over time, the goal is to use focused, one-on-one sessions and your home program to help you make steady progress without relying on unnecessary visits.
Do I need imaging or surgery before starting?
Not necessarily. Many of the people we work with have already had imaging that came back inconclusive, or have been told surgery is the next step. We help you understand what your pain actually means and identify what can be addressed through movement and manual care first. We'll always be honest if your situation calls for a referral.
How is this different from what I did at my last PT clinic?
At most in-network PT clinics, you may see your provider briefly before being handed to an aide for the rest of the session. Here, every minute is one-on-one with your physical therapist. Same provider, every visit. Sessions average one hour, and your home program is updated and accessible through the PTEverywhere app between visits.
What happens on the initial evaluation?
Your initial evaluation is a full hour with your physical therapist. We take a thorough history, assess your movement, identify what's driving your pain or limitation, and walk you through exactly what we found and what we recommend. By the end, you leave with a clear plan, not just a diagnosis and a home exercise sheet.
